Worksheets
EWA/LAO offers two ways to send us your licensing information: online worksheets and downloadable/printable worksheets. Because the FCC requires the applicant’s signature on the Form 601 and the Form 603, it is necessary to send these pages to EWA/LAO either by fax, email or mail. You can also take advantage of our website’s Upload Center to send us scanned copies of your signature pages. Simply scan the completed signature page, save it as a PDF, TIF or JPG file and upload it directly to us (see the Upload Center page for more specific details on file types as well as uploading instructions and restrictions). If you are taking advantage of our online worksheets, you can attach scanned signature page files directly to the worksheets as you fill them out.
Upon receipt of your online worksheets, a EWA/LAO licensing representative will contact you in writing with a breakdown of fees for your application. Payment can be made by credit card (we accept Visa, Mastercard and American Express) by completing and signing our credit card authorization form or by mailing one check for all fees made payable to EWA/LAO.
By utilizing our online worksheets, we can have your application ready for immediate submission by the time we receive the signature page and fees. Or if you prefer, you can download PDF versions of our worksheets and mail or fax them to our office. You will need the free Adobe Acrobat Reader in order to view and print the worksheets.
FCC Form 601 Signature Page
There are actually two pages included in this download: pages #3 and #4 of the FCC Form 601. Page #3 has a number of general certification questions which must be answered by the applicant. Page #4 is where the applicant signs. Both pages should be sent to EWA/LAO. Please enter the applicant name and/or callsign at the bottom of each page so we will be able to easily match them up with your application.
FCC Form 603 Signature Pages
These are the signature pages for an Assignment of Authorization. There are three pages; one page consists of the certification questions which are to be answered by the assignee. Both the assignor and the assignee sign where indicated on the other two pages. Please enter the applicant name and/or callsign at the bottom of each page so we will be able to easily match them up with your application.
Fixed Base/Repeater Worksheets
These worksheets are the most widely used. Anyone licensing a system that employs a fixed antenna structure (i.e. base/mobile, repeater, point-to-point, paging), should complete these worksheets.
Mobile Only Worksheet
This worksheet is for licensing a mobile-only or hand-held radio system. These systems do not utilize a base, repeater or any type of fixed antenna structure.
Site Supplemental Worksheet
Use this form as an add-on to the Fixed Base worksheets when applying for more than one transmitter location. This lessens your paperwork burden and gives us the necessary information on the additional sites. The FCC allows up to six (6) fixed transmitter sites per callsign on Part 90 Land Mobile licenses.
Renewal-Only Worksheet
Don’t let your valuable FCC license expire! Complete this short worksheet and submit it to our office right away to have EWA/LAO file an electronic renewal-only application for you. Part 90 Land Mobile licenses can be renewed up to ninety (90) days prior to their expiration date as per FCC Rule §1.949(a). Note, the FCC Form 601 signature page is not required for a renewal-only application.
Construction Notification/Schedule K Worksheet
Part 90 Land Mobile licensees are required to construct their system within one (1) year of the date of grant as per FCC §90.155(a). The Form 601 Schedule K must be filed electronically to the FCC as per §1.946(d) of the Commission’s rules. Your license copy will reflect a build-out deadline date next to the frequencies. EWA/LAO will be happy to handle this electronic filing for you. Simply fill out our quick online worksheet! The FCC Form 601 signature page is not required for this filing.
Assignment of Authorization Worksheet
This worksheet is used if you wish to assign a license from one entity to another. The FCC Form 603 signature pages will be required, signed by both the original licensee and the new licensee.
