Site Supplemental Online Worksheet
Use this page as an adjunct to the Fixed Base Worksheets should you have more than one fixed transmitter location. The FCC allows up to six fixed transmitter sites per Part 90 license. On the Fixed Base Worksheets you just completed, there is a field at the bottom of the fifth page where you can indicate that you have additional transmitter sites. When we see this, we will know to be on the lookout for a Site Supplemental page with the same licensee/applicant name. Please be sure to enter the same licensee/applicant name at the top of the first page of the Supplemental Site worksheet so we will be able to match up your additional sites with the corresponding Fixed Base Worksheets. If this is a modification of an existing license, please also enter the callsign.
This form consists of five separate pages; one page for each additional transmitter site. Use as many pages as you need for each additional site that you have. For example, if you have two additional transmitter sites, complete pages #1 and #2 and then skip to page #5 to click the Finish button to submit the worksheet. It is okay to leave the remaining pages blank. There is a general comments field at the bottom of each page where you can enter any information or notes about the site that you feel may be helpful.
Online Worksheet Introduction & Instructions
The online worksheet allows you to provide EWA/LAO with all required information electronically rather than through printed copies, mail or fax. You can click on the Help icon 
that you will see in various places throughout the worksheet for more detailed information on specific points. (Note: If you use popup blocking software, you may need to adjust it to allow popups for this website, since the Help feature utilizes small popup windows to provide information, tips and explanations.)
The online worksheet consists of multiple pages. You can proceed from page to page in sequence by clicking the Next button at the bottom of each page or you can use the page navigation buttons 123 to access any page you wish in any order. Feel free to make changes and corrections to your information at any point and be sure to review all pages of your worksheet carefully before final submission.
Do NOT use your browser's Back or Forward buttons while navigating the online worksheet or you may lose some of the data you have input. Always use the page navigation buttons or the Next buttons. Your browser must be of relatively recent vintage (we recommend Firefox or Opera) and must support JavaScript (most browsers do) in order to use the online worksheets.
When you have completed your worksheet, click the Finish button on the final page to submit it to EWA/LAO. Upon submission of your worksheet, you will receive an acknowledgement via email.
